If you’re passionate about delivering responsive, person-centred care and thrive in a fast-paced, evolving environment, this role offers genuine impact and variety.About This RoleThe post holder will provide expert clinical oversight to community-based complex care packages across multiple locations in and around Herefordshire & Worcester and will be expected to travel to service users’ locations for staff competency sign-offs and quality assurance.You will be involved in carrying out initial assessments and setup up new care packages, ensuring safe, responsive, and person-centred care aligned with regulatory and best practice standards.Responsibilities:Maintain clinical oversight of high-risk or complex care packages, ensuring safe, compliant, and evidence-based delivery.Conduct regular reviews and competency checks on-site across all active locations.Identify and escalate emerging risks; contribute to lessons learned and audits.Participate in new care package assessments alongside the MDT.Support in ensuring care plans, risk assessments and staffing allocations are clinically appropriate from the outset.Review specialist clinical tasks where required.Travel to service user homes to complete hands-on staff training and sign-offs for clinical competencies (e.g., PEG, tracheostomy, suctioning).Provide clinical supervision and mentorship to RGNs and care staff, ensuring adherence to professional standards.Contribute to rota reviews and highlight clinical skill gaps to coordination or compliance teams.Experience RequiredRegistered General Nurse (RGN) or Registered Children’s Nurse (RCN) with valid NMC registrationSignificant post-registration experience in complex care, community nursing or clinical leadershipStrong understanding of CQC regulations and clinical governanceConfident in lone working, travel, and making rapid decisions in community settingsClean driving licence
